Step 1: Click the "Book Now" link below to choose your appointment type, date, and time.You will receive a notification that you are scheduled however you will need to do step two which requires you to fill out the paperwork.
Step 2: After you have booked your appointment, you will need to fill out the paperwork here and complete the highlighted areas. Once completed and submitted your appointment will be confirmed. Please note that a credit card is used ONLY to hold your appointment. Payment is due at the time of services
Step 3: Once you have completed the required electronic paperwork, you will then receive an e-mail from Echosign. Check your e-mail inbox, open the e-mail, and click on the provided link to confirm your appointment
Please Note: Appointments are not confirmed until you have submitted the REQUIRED electronic paperwork and have clicked on the confirmation link that has been sent to your e-mail!
Please fill your paperwork out right after you've booked an appointment.The system is coded and will cancel your appointment after 1 hour.
